Equine Re-Homing Questionnaire

If you are an owner and want to submit your equine for surrender and re-homing to Rosemary Farm Sanctuary, please complete the following questionnaire. This form was created to streamline the weekly requests we receive to take in equines. Some questions may not pertain exactly to your situation; please answer as best possible. The information will help our team understand the equine and the situation so we can determine if and how we may be able to help. NOTE: If you are re-homing more than one equine, submit a separate form for each. ALL information is required for us to consider this request.

A bit about us… Rosemary Farm Sanctuary is an established 501(c)3 dedicated to helping equines in need and protecting them for life. There are currently ~100 equines in our care, of a wide variety of breeds, backgrounds, ages and circumstance. Some may be adopted into approved homes with a contract, but the majority remain at our facility. We are an ‘at will’ rescue and only accept equines that we have space and resources for, and that meet our intake criteria. Our program is supported by private donors; there is no government funding and no tax dollars to help. We are not a free retirement program; we focus on dire need cases. We are not an agent of the law and cannot seize equines or conduct searches or investigations; possible abuse or neglect cases must be brought to the attention of your local SPCA or law enforcement first. We are happy to work with law enforcement agencies if a case requires a safe surrender location.

Our team will review your submission as soon as possible, which may take up to a week. We may have follow-up questions by email and may require a phone conversation. We strive to provide quality, long term care for the ~100 equines that are already part of Rosemary Farm Sanctuary and any others that we make a commitment to.

If you prefer to complete the form(s) offline you may download the Equine Re-Homing Questionnaire and submit the completed form and photos by email to info@rosemaryfarm.org.
